Dear 4/5 Multiage Parents/Guardians,
On October 21, 2011 we would like to be traveling to Hannaford Supermarket on Union Street (a separate permission slip will soon be coming home once we know we have enough drivers). We will need parent drivers to transport us. We will be leaving school around 9:15 AM and returning around 11:00 AM. If you are able to help please fill out the form below and send it back to school as soon as possible so we can make necessary arrangements.

ALL PARENTS ATTENDING THIS FIELD TRIP NEED TO MAKE SURE THEY HAVE VOLUNTEER CLEARANCE & THAT WE HAVE YOUR DRIVER'S LICENSE NUMBER IF IT IS NOT ALREADY ON FILE. Many of you are already “cleared” but some have not yet completed the necessary forms.
